I've had two shutters go - one on a used 1D I purchased from a journo ( to be expected I guess) and another on a MKIIN after only 96K actuations (Canon's claim is - "good for 200K").
I have two 1D MkII's and have had the shutter replaced on one of them twice and the other once. At around 100-110k activations each.
I had two shutter failures on my 1DMkII (66k and again at 110k) and a failure on my 1DMkIIN at 69k. Even though the 1DMkII was a few weeks out of warranty on the first failure Canon replaced the shutter at no charge.
My 40D's shutter just died 2 months out of warranty and all it had done is an unacceptable 14K clicks (Canon claim 100K).
It just cost me AU$700 to have a shutter replaced on a MKIIN (96k actuations) in Sydney.

You need a new shutter. That is classic shutter bounce. The repair is about $200 if you are out of warranty.
D700: Had mirror failure on mine at 55k shutter actuations - it got locked in "down" position and wouldn't flip up when shutter release button was pressed.
